EEOC Notice Regarding EEOC Facilities COVID-19 Screening

In order to protect the health of EEOC employees and contractors (hereby referred to as staff), all EEOC staff must self-screen for symptoms or other risk factors of COVID-19, using EEOC’s COVID-19 screening tool on their mobile device or on a paper form at designated locations at facility entrances, prior to admittance to EEOC facilities. Staff must use the tool to provide confirmation to the designated EEOC representative that they are approved for access before being granted access to EEOC facilities.

EEOC staff must answer the following questions and then certify their answers are true and correct in EEOC’s COVID-19 screening tool. All responses to the following questions should be "yes" or "no":

• Have you experienced any of the following symptoms in the past 48 hours: fever or chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, diarrhea?
• Have you been in close physical contact in the last 14 days with anyone who is known to have laboratory-confirmed COVID-19 OR anyone who has any symptoms consistent with COVID-19?
• Are you isolating or quarantining because you may have been exposed to a person with COVID-19 or are worried that you may be sick with COVID-19?
• Are you currently waiting on the results of a COVID-19 test?
• Have you traveled in the past 10 days?

If you answer "yes" to any of the above questions, you will be denied entry to EEOC facilities and you will be provided further instructions—either through the tool or on paper.

If you decline to self-screen using EEOC’s COVID-19 screening tool, you will be provided the option of completing a paper version. If you decline to self-screen using either option, you will be denied entry to EEOC facilities.

If you are a EEOC employee or contractor who is denied entry, you must speak with your immediate supervisor or contracting officer’s representative (COR) and/or management company about options.

Additional Information

• No personally identifiable information will be recorded.
• If you have a chronic medical condition that causes COVID-19-like symptoms and you need to access a EEOC facility within the next few days, please contact your supervisor. If you do not require access to a EEOC facility but have concerns about your symptoms, please contact your primary care provider.
• If you are a contractor who is denied entry, you should contact your contracting officer’s representative (COR) and/or management company for further guidance and direction.
